Hi Morgan here from Form Approvals support. As the earlier response from another customer suggested, just make sure the Google Forms page has completely loaded before trying to open an add-on. Add-ons are unfortunately the last item to load on the page, so it can cause only the Help menu item to display when trying to open the add-on too quickly.
